Evaluation tools have been created for select 4-H programs happening statewide. In addition, select Issue Teams implementing 4-H programming have made their evaluation instruments available. All of these tools were specifically developed to measure the outcomes targeted by each program. All staff delivering these programs should plan to facilitate the supporting survey.
In addition to the evaluation instruments available below, the National 4-H Common Measures instruments are designed to assess the impacts of 4-H programs in science, healthy living, citizenship, college/career readiness, and universal positive youth development.
Recovery Service Learning NEW
Please use the following instruments with your 4-H and adult volunteers to assess the impact of their Recovery Service Learning experience.
Adult Survey
Youth Survey
4-H Statewide Program Evaluations Instruments
Data from these programs will be collected until October 1st.
Please review the Collection Guidelines before administering the survey. Youth can complete any of these surveys online through the use of a custom survey link, or the surveys can be printed and completed on paper.
Resources needed to conduct evaluations can be found below. There you will find collection guidelines, a program cover sheet, information letter (we still have a waiver for consent meaning no signature is required, this letter provides information and details regarding the study), and the various evaluation instruments.
